ОСОБЕННОСТИ ВЫПОЛНЕНИЯ ТРАНСПЛАНТАЦИИ ТРУПНОЙ ПОЧКИ
Аннотация
Цель. Снижение риска ранних послеоперационных осложнений при выполнении трансплантации трупной почки, связанных с нарушением кровоснабжения донорского мочеточника. Материал и методы. Проведен анализ осложненного течения послеоперационного периода, связанного с особенностями кровоснабжения донорского мочеточника у 20 из 410 (4,9%) пациентов с хронической почечной недостаточностью, которым проведена трансплантация трупной почки в условиях Брестской областной клинической больницы с 2011 по 2020 гг. Результаты. Разработана методика определения анатомических ориентиров, указывающих на расположение сосудов, питающих донорский мочеточник. Освоение специфических технических приемов, применяемых во время подготовки почечного аллографта и его имплантации, позволило снизить количество урологических осложнений после выполнения трансплантации почки более чем в 2 раза, количество случаев послеоперационных некрозов мочеточника – в 4 раза. Выводы. Предложенные хирургические технические приемы позволяют уменьшить риск послеоперационных осложнений при выполнении трансплантации трупной почки, связанных с нарушением кровоснабжения
донорского мочеточника.
