УЛЬТРАЗВУКОВОЕ ИССЛЕДОВАНИЕ ДИАФРАГМЫ КАК ПРЕДИКТОР УСПЕШНОГО ОТЛУЧЕНИЯ ПАЦИЕНТОВ ОТ АППАРАТА ИСКУССТВЕННОЙ ВЕНТИЛЯЦИИ ЛЕГКИХ
Аннотация
Актуальность проведенного обзора обусловлена сохраняющейся проблемой отлучения пациентов, находящихся на аппаратах искусственной вентиляции легких длительное время. Цель. Проведение литературного анализа для обобщения результатов современных исследований по изучению сонографических параметров, позволяющих оценить диафрагмальные функции для прогнозирования успешного отлучения пациентов от искусственной вентиляции легких. Материал и методы. Проанализированы 33 русскоязычных и англоязычных источника. Результаты. Подробно рассмотрены следующие ультразвуковые показатели диафрагмы: экскурсия диафрагмы, фракция утолщения диафрагмы, диафрагмальный индекс быстрого поверхностного дыхания, время достижения пиковой амплитуды диафрагмы на вдохе. Выводы. Умение вовремя диагностировать дисфункцию диафрагмы с помощью ультразвукового исследования минимизирует риск развития дыхательной и сердечной недостаточности. В настоящее время существуют методы стимуляции диафрагмальной мышцы для ускорения восстановления утраченных функций.
