TREATMENT OF MALIGNANT MCA INFARCTION

Аннотация

Злокачественный инфаркт мозга в бассейне средней мозговой артерии, сопровождающийся объемным эф­фектом, является одной из наиболее деструктивных форм ишемического инсульта. Несколько исследований се­рий случаев предложили декомпрессивную гемикраниэктомию в качестве сохраняющей жизнь операции, но до недавнего времени не было доказательств пользы данного вмешательства, полученных в рандомизированных контролируемых испытаниях. Данные, опубликованные в 2007 году и результаты объединенного анализа трех европейских испытаний за 2009 год установили факты в пользу гемикраниэктомии. Данный обзор посвящен клиническому синдрому, содержит информацию по патофизиологии и способам визуализации, а также описы­вает лечебные подходы с особым вниманием по отношению к декомпрессивной хирургии.
