ЭНДОСКОПИЧЕСКИЕ ТЕХНОЛОГИИ В ТИРЕОИДНОЙ ХИРУРГИИ: МЕЖДУНАРОДНЫЙ И СОБСТВЕННЫЙ ОПЫТ

Аннотация

Актуальность. С развитием эндоскопической техники широкое развитие получили малоинвазивные вмешательства на щитовидной железе. Введение в практику малоинвазивных методик связано со стремлением улучшения косметического эффекта и уменьшение операционной травмы для пациентов. Цель исследования: изучить наиболее популярные методики, используемые в миниинвазивной хирургии щитовидной железы, показать преимущества и недостатки каждой методики, вероятность возникновения того или иного послеоперационного осложнения в зависимости от метода хирургического вмешательства на щитовидной железе. Материал и методы. Из малоинвазивных вмешательств широкое распространение получили минидоступы: подмышечные, передние грудные, параареолярные, позадиушные, трансоральные. При выполнении видео-ассистированных оперативных вмешательств используются эндоскопическая стойка, стандартный набор инструментов (зажимы и ножницы для эндоскопических операций), ретракторы для создания операционного пространства. Результаты. Обоснованы критерии дифференцированного подхода к выбору метода миниинвазивного оперативного вмешательства на щитовидной железе. Основная задача при выборе метода оперативного вмешательства – его простота, возможность быстрого освоения, экономическая эффективность, безопасность. Выводы. Обеспечение безопасности и правильной техники выполнения оперативного вмешательства с обязательным мониторингом возвратного нерва, визуализацией паращитовидных желез позволил получить хорошие клинические результаты у пациентов с данной патологией.
